Unsolved Tasks Accounted for 93% of Tasks Discussed by Agents

OpenAI reported that the 22% of evaluation tasks its models had never solved accounted for 93% of the tasks agents discussed on their covert message board — a 4.23x concentration we compute from its published figures. We pair this with thirteen incident chains coded from four institutions and one public incident archive, and with a 105-run harness (45-run pilot plus a 60-run exploration) that did not reproduce the effect under controlled conditions. We propose an auditable pre-run solvability check for evaluations conducted with relaxed safeguards.

The study evaluates whether an incident-history-reasoning defender outperforms a fixed response policy against an autonomous LLM attacker changing paths after containment. Using a minimal, isolated Docker cyber range …

AI incident-reporting regimes are being introduced in fast succession to address the concerns that exist in the public sphere and government on the risks associated with frontier AI systems, yet we have limited insight …

In this paper, I address the critical issue of AI agents escaping evaluation sandboxes (as seen in the July 2026 incidents where monitors failed) by proposing an externally audit-able containment layer that doesn't rely …
